Account Check List

The following information is required for opening an account with SDS Vend Supply.

  1. A copy of your STATE Sales Tax Certificate** (or your states equivalent)
  2. A completed W-9 form.
  3. A signed information release form for bank and references.
  4. To review payment’s by check we need a bank and account number with a phone number for your branch. (NOTE: we do not pay or participate in fees for credit references, if this is an issue please discuss options with your branch)
  5. References need to offer credit information for evaluation.
  6. If you are interested in paying in advance by check. Check must clear account before order can be released for scheduling and shipping of products. Wire transfers are accepted for immediate order release.

Please be aware that this information is kept confidential and will only be released by request to authorized state and federal agencies. 

Pease note that Omission of any of the Required forms “STATE Sales Tax Certificate**” and/or  “Federal W-9 form”  Will Delay your customer account from being established. These forms are required to be in your customer file.
